Subject: epub, html, odt: namespace names as identifiers

namespace names are identifiers, not addresses, fixes
A namespace URI is an opaque identifier.
  www.w3.org        xhtml, xlink, MathML, rdf-syntax-ns,
                    xml-events, XMLSchema, XMLSchema-instance,
                    xforms, css3-text, data-view, and the xhtml1
                    strict DTD system identifier
  purl.org          dc/elements/1.1, dc/terms
  openoffice.org    office, writer, calc, table, report
  docs.oasis-open   the ODF meta pkg and odf class identifiers
